Over-Charge perhaps on a Cell, it can cause swelling, I belive when you charge somthing (if using hobbyking balance charger) to use the Balance option After fully charged, to my understanding.... it just evens out the voltages per cell to an acceptable level.

Best keep as eye on that battery, its performance will surely be reduced

No. 2200 is the mah rating...meaning capacity. 3S means 3 cells or 11.1v battery. You want to stick with 1800-2500mah else the weight and size of the larger loop might not fit your model
